• Medical Consultants
• Medical Officers
• Dental Surgeons
• General Practitioners
• University Academics (Medical)
• Retirees of above professions

Procedure of Obtaining the DOWELS Membership

Applications are available in DOWELS office at TH Kandy or SBSCH.  If you are unable to collect the application from the office, you can download it from the above link.

Please hand over your correctly filled application to DOWELS office at TH Kandy or SBSCH.

• Joining fee (life time): Rs. 2000.00 payable only by cash to DOWELS Office.
• Monthly membership: Rs 750.00 have to give a standing order  through BOC / Sampath / HNB / Commercial / Peoples'  Bank ( Preferably BOC)
